AUSABLE MARTIAL ARTS ACADEMY, LLC. - MEMBERSHIP AGREEMENT
STUDENT MEMBERSHIP AGREEMENT
- I agree that the Membership Fee is due the first week of each month.
- I agree that should the Membership Fee be late, a late fee of $10 will be charged.
- I agree to the Promotion Test Fees: $50 White Belt through Blackbelt / $150 Shodan Blackbelt.
- I understand that under the terms of this agreement, the AuSable Martial Arts Academy will furnish me a competent instructor and proper facilities for the teaching of lessons. It is further agreed that my failure to take all lessons contracted for shall not relieve me of my obligations to pay the total tuition herein agreed upon. I further understand that no refunds for any tuition paid shall be made due to absence or withdrawal.
- I agree that all exercises and workouts shall be undertaken by me at my own risk and I will not hold the AuSable Martial Academy responsible for injuries that may result in training. The AuSable Martial Arts Academy and its instructors have authorization to approve any emergency medical care that the above student may require if a parent and/or guardian is absent. I will not hold the AuSable Martial Arts Academy responsible for any loss, damage, or theft of any item personally belonging to me, while at the school or attending any AuSable Martial Arts Academy activity.
- For consideration herein acknowledged as received, and by signing this release, I hereby give the photographer/filmmaker and assigns my permission to license content and to the content in any media purpose (except pornographic or defamatory) which may include, among others, advertising, promotion, marketing and packaging for any product or service. I agree that the content may be combined with other images, text, graphics, film, audio, audiovisual works, and may be cropped, altered, and/or modified.
